Purpose of Assignment:

The purpose of this paper is to define a clinical problem, review the literature for appropriate research supported nursing interventions, critique current practice and provide a plan for changing or adapting clinical practice
Student Approach to Assignment:

For this assignment, I reviewed my current workplace nursing interventions and determined a current clinical issue. This clinical problem was reviewed and evaluated. I addressed the factors contributing to the issue and used research to formulate a plan for change. With this project, I learned that not all nursing interventions are being carried out as they should. Also, I gained a better understanding of the benefits that accommodate thorough research. It is vital to know the origin of any clinical issue so it can be solved promptly and efficiently.

This assignment should be included in my portfolio because it showcases the importance of implementing change in the workplace. This assignment represents me as a nurse by displaying the research, time and effort dedicated to improve patient safety and quality of care.

After conducting research, it is proven that SHRH scored below average for Leapfrog’s “Staff working Together to Prevent Errors” section (The Leapfrog Group, 2018). Nurses must work together to ensure that they are administering the correct drug in the right dose, to the correct patient, in the right route, at the correct time and be sure to document afterwards. It was also determined that this hospital scored below average for working together in an effort to prevent errors. Actions are revised based on current data; current data implies that new nursing actions that are aimed at reducing medication errors should be utilized. For instance, nurses at SHRH are do not receive mental. The major discrepancy between the formal and informal mechanisms is the complete negligence of performing the six rights of medication administration. The reasoning for shortcuts is to reduce the amount of time spent giving medications so nursing care can be applied in other ways. In short, due to the chaotic world of healthcare, nurses are finding alternative practices to preserve time. The act of not performing safety checks should be revised immediately. Supported by research, time management and burnout prevention interventions would greatly reduce medication errors.

In this written assignment, I proposed a strategy to reduce the occurrence of medication administration errors. My strategy for reducing the occurrence of medication errors would include a 30-day educational seminar for the nursing staff that reviewed the six rights of medication administration. This seminar would discuss the most common type of medication errors that have occurred and what can be done to reduce those. There would be a pre-test and post-test to gauge the effectiveness of the educational intervention. Teamwork would be emphasized during the meetings, as it is a responsibility of each nurse to ensure patient safety.

I am aware of the potential health problems associated with medication administration errors. Providing in-depth educational sessions for both newly graduated and experienced nurses will help to mend any knowledge deficit that is causing the medical errors. Burnout prevention strategies such as journaling are healthy habits to utilize early on, as this creates a positive outlet for the accumulating stress and tension-both which lead to the clinical problem at hand. Lastly, performing safety checks will be the most valuable intervention implemented because verifying every aspect of medication administration will monumentally reduce the occurrence of medication errors. I apply each research-driven intervention to guide my nursing practice.

It is beneficial that nurses engage in complementary modalities, such as journaling to reduce burnout and promote health.

Baker & Weant (2014) found that “having an insufficient knowledge base pertaining to medication use has been noted as a common root cause of medication-prescribing errors.” After their three-month educational session, there was an improvement in two categories of education errors studied. For example, the “IV fluids ordered but not given” category decreased from 4.9% to 1.4% and the “Incomplete Documentation” section dropped from 14% to 7.4% (Bailey, Baker & Weant, 2014). In my current nursing practice, I am sure to check IV lines and label them correctly so medication errors are less likely to occur.
