Purpose of Assignment:

The purpose of this assignment is to provide the student with the opportunity to summarize, in detail, the development, implementation, and evaluation of an instructional intervention for a specific client or professional group.
Student Approach to Assignment:

I approached this assignment by conducting research on a relevant clinical practice problem. Then, I determined the learning needs of my anticipated audience, which consisted of first-year nursing students. Through this assignment I was fortunate to have the opportunity to experience the value of education in the world of healthcare. After conducting my educational session for my intended audience, I was able to achieve my goal of spreading awareness about the clinical practice problem at hand by developing, implementing and evaluating my own instructional intervention. This assignment reflects my personal values by showcasing the importance of conducting thorough research on an issue and actively working towards interventions that will help to prevent its occurrence. I value the diligence of evidence-based research and practices that improve healthcare and the well-being of patients. I chose to approach this assignment in a research-driven method because the most effective way in reducing the occurrence of medication errors is to be knowledgeable and aware of what causes an individual to make those errors.
Reason for Inclusion of this Assignment in the Portfolio:

This assignment should be included in my portfolio because it showcases the research, development and evaluation of an instructional intervention implemented to help prevent the occurrence of a clinical practice problem, medication administration errors. Not only did the development of an educational session help highlight the importance of education in nursing but it also helped to implement key workplace changes that can be applied to help reduce the occur of medication administration errors.
Critical Thinking

o Uses nursing and other appropriate theories and models to guide professional practice
As the educational designer, reviewing educational theories is important because they help guide the reasoning behind learning. For this project, I applied the principles of the cognitive theory to my educational session, as it states that the learner must find the experience meaningful in order to change or benefit. As this theory guided my development for the instructional intervention, I conducted a thorough assessment of my target audience. I used observational surveys and created an educational needs survey, assessing the learning style, perception and level of knowledge pertaining to the issue, interest in and (convenient) time of educational session, preferred educational formats and motivation for attendance of the session. I also apply this theory in my clinical setting, through asking my patients what they know about their illness or medical condition. Also, I evaluate how they perceive their illness or how their medical condition may affect their lifestyle (For example, do they consider their condition serious, or are they making not medical advice taking advice seriously)

Nursing Practice

To evaluate the attainment of each objective, I used question and answer, observation and posttest methods. During the game/simulation, I observed each student correctly administer two medications, finding the three purposeful mistakes hidden throughout the scenario. The quiz score average was 100%, with every member of the audience listing three correct consequences of medication administration negligence. Each learner correctly verbalized all six rights of medication administration and discussed five major concerns relating to medication administration.
For the evaluation of the overall effectiveness of instruction and educator, I created a post-session evaluation survey. Four out of the five attending members selected the highest satisfaction rating (#5, “exceptional”) for the first question (see appendix D for survey); the remaining member chose the number four. All five members selected “yes” to the question “Was your instructor informative?” For the overall satisfaction of the educational session, all five members selected the highest rating (#5, “very satisfied”). Each member selected “yes” to the question “Did you learn any valuable information in preventing medication errors?” The instructional goal was met and the outcomes were parallel to the objectives determined early on

Teaching

o Provides teaching to patients and/or professionals about health care procedures and technologies in preparation for and following nursing or medical interventions
Through this assignment, I was able to research a clinical practice problem and develop an instructional session for a group of nursing students. The content was centered around the current healthcare issue and the purpose behind the teaching session was to provide nurses with the necessary information to properly administer medication. The goal of the teaching session was to have nurses independently administer medication using all six rights. For example, I lectured on each of the six rights of medication administration (right patient, right drug, right time, right route, right dose, right documentation) and then guided the students to the simulation lab to practice medication administration on the mannequins. Today, I utilize those same six rights when giving medications to my patients.
Research

o Shares research findings with colleagues
In-depth research was conducted and data supports that the biggest concern within Sentara Halifax Regional Hospital (SHRH) was inadequate medication administration practices. According to Leapfrog’s Hospital Safety Grade of 2018, Sentara Halifax Regional Hospital (SHRH) scores a C grade. Within that score, SHRH scored below average for the “Staff working Together to Prevent Errors” section.

Reviewing the scopes and standards for Acute And Critical Care Nursing (AACN) practice reiterates the importance of maintaining the highest individual performance measurement as possible (The American Association of Critical Care Nurses, 2010). Standard two is implemented to increase the performance of the work staff, in efforts to create the best nursing experience obtainable for patients. It is within the nurse’s scope of practice to uphold the rules and regulations of medication administration regardless of time constraints and/or distractions.

The content was centered around the current healthcare issue (see Appendix C). The purpose behind the teaching session was to provide nurses with the necessary information to properly administer medication. The goal of the teaching session was to have nurses independently administer medication using all six rights. The materials used were based on the results of the educational survey; most members of the audience were active learners and preferred lectures, PowerPoint presentations, gaming and/or question and answer (Q&A) sessions. Simulation/gaming were great instructional methods for the active learners because it engages/stimulates the learner. To evaluate the effectiveness of my educational sessions, I monitored and delegated the nurses during medication administration in the simulation lab. I was responsible for the accuracy of administration, as this was a direct reflection of the effectiveness of my educational seminar.
