NURS 492 – Community Health Nursing

 

Purpose of Assignment:

 

The purpose of this clinical application assignment for the student to refine the planning process, implement and evaluate interventions addressing the health needs of the chosen population

 

Student Approach to Assignment:

​

I approached this assignment by using information gathered in part I of my community health project. During part I of this two-part project, I selected my target population, conducted a windshield survey of my community, reviewed and compiled research data that guided me towards a prevalent health-related issue. It was decided that Diabetes was the most prevalent disease/healthcare issue in my community. I reviewed the data from part I and used the nursing process to implement my therapeutic interventions for the population of Dan River Church. I identified a priority nursing diagnosis, appropriate interventions and developed a plan for evaluation that addresses achievement of objectives.

 

 

Reason for Inclusion of this Assignment in the Portfolio:

This project should be included my portfolio because it showcases the dedication and work ethic contributed to implementing therapeutic interventions that help benefit the community. This assignment represents me as a student and nurse in the way it highlights my passion for community nursing; providing education and evidence-based interventions to help prevent the occurrence of Diabetes within my community is something I am most proud of.

Teaching

​

o Provides relevant and sensitive health education information and counseling to patients, and families

On the second slide of my presentation I address the top health concerns for my community (Halifax County). For this project, I arranged for weekly meetings at Dan River Church. There, I discussed the risk factors for diabetes mellitus, the cause for diabetes and counseled the members whom needed extra advice; I answered questions and provided individuals with educational brochures.

 

 

Leadership

  • Assumes a leadership role within one’s scope of practice as a designer, manager, and coordinator of health care to meet the needs of populations

 

As the designer and leader of this community health project, I was solely responsible for creating the PowerPoint presentation, which was equipped with adequate research.  I formulated therapeutic interventions, such as holding one-on-one educational sessions on how to properly use a glucometer and created “pop quizzes” to evaluate the effectiveness of my educational sessions. I organized each meeting and constructed flyers to alert my target population of the time/date of the meetings.

 

  • Initiates community partnerships to establish health promotion goals and implements strategies to meet those goal

 

To complete both parts of this project, I had to reach out and communicate with my target population.; the members of my population attended Dan River Church. I spoke with Mrs. Floyd (an active member of the church) and a few other attending church members to discuss their willingness to participate in my community project. I explained in detail the parameters of the project and their eagerness to participle in meeting the established health-related goals. We worked as a team to become more knowledgeable about diabetes and decrease the amount of modifiable risk factors.

 

  • Organizes, manages, and evaluates the development of strategies to promote healthy communities

 

With this assignment, I reviewed the most prevalent healthcare issue for my target population; this issue was diabetes mellitus. I developed a strategy of holding weekly educational sessions on diabetes to promote healthy communities. I created “pop quizzes” and evaluated the effectiveness of my strategies.
